How to prepare for your first visit

2.Complete all sections
Fill out your personal information, medical history, insurance details, and sign the consent forms.
3.Bring to your appointment
Bring the completed forms along with your insurance card and a valid photo ID to your first visit.

What to bring to your appointment
- Completed new patient forms
- Valid photo ID (driver's license or state ID)
- Insurance card(s)
- List of current medications and dosages
- Any relevant medical records or test results
- Co-pay (if applicable)
